Protect Your Finances against Critical Illness with Sun LifeAssure

What is it?

Aside from VUL (Insurance with Investment) and term plans that I have, I also get Sun LifeAssure because if I am diagnosed today with any of the 36 critical illness (wag naman sana), Sun Life will give me Php500k for my medical treatment and hospitalization expenses provided I will submit all the necessary documents like doctor’s certificate and documents from the hospital.

What if I can’t file the documents myself due to paralysis? I can ask someone to do that, a duly appointed guardian to file and receive the benefit amount on my behalf.

How much is the cost?

If a 30 year old male, non-smoker wants to have a benefit amount of Php500K, he will pay Php5,140/annually or Php2,724/semi-annually.

If a 30 year old female, non-smoker wants to have a benefit amount of Php500K, she will pay Php5,070/annually or Php2,687.10/semi-annually.

The sample rate includes additional riders such as: 

        Accidental Death benefit – amounting to Php500K, if the cause of death is accident

        Total Disability benefit – waiver of premium

NOTE: The minimum coverage is Php500K and maximum coverage is Php4 Million. Premium increases every five years and renewable every year. It doesn’t have any cash or fund value.

How to file a claim with Sun Life?  

Here are the Basic Claim Requirements:

1) Claimant’s Statement

2) Attending Physician’s Statement

3) Authorization (form may be secured from Claims Services Department of Sun Life of Canada (Phils.), Inc.

4) Admitting History or its equivalent inclusive of other hospital records containing patient’s past medical history

5) Discharge Summary or its equivalent

If critical illness is caused by an accident or violence

Police Report

Medico-Legal Report
